The device didn’t boot.
After boot in to recovery and formatting the data partitioning the FP3 boots the previously installed /e/OS from slot a.

The flash-all scripts should check the bootloader version string, because with

it definitely NOT work at all.

To the issue, I think it isn’t exactly that simple (checking if version-bootloader contains a version or not).
On your graphical install log, there is this:

So that could be a connection issue, etc.
But not sure why your manual install that reported success then idn’t work. Maybe it didn’t switch the slot or something, there was a complaint about that on the ?FP4? recently?
iode is always installed in slot ‘a’ by default. After installation the slot is active, but didn’t boot.
I think with the latest FOS for the FP3 (with the new dark green ‘Fairphone’ logo and version-bootloader ‘6.A.039’) came some changes in the boot chain, that causes this problems.

Previously installed stock image FP3-6.A.040.2-gms.
The graphic installer seams to use a similar fastboot sequence as the CLI script.
Simply check the output of
$ fastboot getvar version-bootloader
version-bootloader: 6.A.039

Obviously the bootloader version string only exists in the 2 latest releases of FOS.
The error only occur on devices with the boot splash looks like this:
Maybe I have a device available with the older bootloader next week.
Update: I received another FP3, but it also have a bootloader 6.A.039. Without using edl no chance to set up a clean test environment.
The culprit part is that line at the end of the flash script: ‘$fastboot erase userdata’. Once removed, everything’s fine. Another solution, with the original script, was to format userdata in recovery before rebooting. The fastboot package script has be modified accordingly.
The current iode-6.15-20260506-FP3-fastboot.zip.sha256 does not match.
It looks like vincent just updated it yesterday, thanks for noting that.
